In order to assist the reporting process and to ensure the most appropriate response to your report, please utilize the following chart to determine which reporting option fits your needs.
In all cases, if you believe there is an immediate or ongoing threat to the safety of an individual or campus community, call 911. Otherwise, simply fill out and submit the appropriate form.

Crime Report
Use if you need to report that you have witnessed a crime, or if you know of a crime that was committed or will be committed.
Please note this reporting option is NOT intended for crimes or for emergencies in progress. If you believe there is an immediate or ongoing threat to the safety of an individual or campus community, call University Police at (203) 932-7070 or 911.
Otherwise, simply fill out and submit the form. A member of the University of New Haven Police Department will investigate the information provided.- If you are a Campus Security Authority (CSA), you must include your name and contact information in the event follow-up is needed. The purpose of this form is to provide information to the University Police Department for the determination of a Timely Warning, Emergency Notification, and to provide federally required data for the Annual Security and Fire Safety Report. Other members of the campus community may choose to complete this form anonymously.
- Example: You recently learned that a student in your class missed their final exam because they were the victim of a robbery on campus. You would utilize the crime reporting form to inform University of New Haven PD that this crime took place on campus.

General Incident Report
If you have an immediate safety concern for yourself, or you feel the student/community member needs medical attention or poses an immediate threat to themselves or others, please contact University Police at (203) 932-7070 or 911.
Use this report to describe incidences of concerning behavior, discriminatory actions, violations of University policy, and/or other concerns within the University community. Upon submission of the form, you will receive an email confirming its receipt to designated officials, such as the Dean of Students Office, University Police, or the University's Behavioral Intervention Team.
Concerning behaviors that can be considered out of the ordinary can take place in and out of the classroom, as well as on and off campus. It is important to remember that early intervention of such behavior(s) is crucial. Examples may include significantly disruptive behavior, sudden/drastic change in demeanor, destructive behavior, or property damage, acts of violence, and/or possession of a weapon.
Discriminatory behavior/actions is any behavior that involves an expression of hostility against the person or property of others because of traits related to their identities or other category protected by state or federal law.
Violations of University Policy refers to any actions that have allegedly occurred within the University's community which would violate the established policies in the Student Handbook, Employee Handbook, or Faculty Handbook at the time of the incident.
Any other incidents of concern that may have occurred within the University's campus or have been perpetrated by University community members may also be reported through this form.
Your privacy and trust are very important to us. Every effort will be made to respect your privacy, however, please be aware that in circumstances involving the compromising of community safety or criminal action, it may not be possible to guarantee anonymity.

Academic Integrity Report (Instructor Only)
Consistent with the Academic Integrity Policy, when an instructor suspects, or receives an allegation, that a student has engaged in an act of academic dishonesty they may complete the Academic Integrity Online Submission Form. Any supporting documentation should be electronically attached to the online form. Alternatively, a description of the violation and supporting documentation, if any (e.g., a copy of the assignment with plagiarized passages identified), may be provided to the Dean of Students at deanofstudents@newhaven.edu. Instructors are encouraged to review the Academic Integrity Policy and its grievance procedures prior to submission of this form. Note that violations discovered by an instructor/supervisor more than one year after the time of the alleged violations might not be subject to formal proceedings.

Therefore, we highly encourage all community members to use the form they feel most comfortable completing in instances where the incident may not clearly fall within the above categories.
